Loss of Coverage Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in up to 30 days after losing your alternative coverage.

Before completing this form you must provide documentation such as a letter from the alternate insurance carrier or the employer stating the student's name, insurance carrier's name and policy number and the loss of coverage date by emailing ccssplan@camosun.ca

If this deadline is missed, you will be eligible for an Anniversary Re-Enrol.

Anniversary Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in each year on the anniversary of your enrolment.

You have 30 days from the anniversary of your program start date to submit the re-enrolment form.

If this deadline is missed, you will not be eligible to opt in again until next year, following the previous guidelines.

Accessibility Accommodation Opt-In

If you're enrolled part-time, but meet certain accessibility requirements, you can submit the opt-in form up to 30 days from your program start date.

You must be taking a reduced course load (a minimum of 36 hours in the first seven weeks of a semester) due to a physical or mental condition. You can reach out to your plan support office at ccssplan@camosun.ca for more information.

If this deadline is missed, you will not be eligible to opt in again until next year, following the previous guidelines.

  1. All new, eligible students go through auto-enrolment in the Health Plan and do not need to submit a re-enrolment form. You will see the Plan Fee(s) on your student account to confirm you are enrolled.
  2. Students can choose to re-enrol in the Health, dental or both plans.
  3. Approval of the Health and Dental Plan re-enrolment application will result in the Health Plan Fee and/or the Dental Plan Fee being assessed. You will receive an email explaining how to pay the fees.
  4. NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
