Loss of Coverage Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in up to 30 days after losing your alternative coverage.

Before completing this form you must provide documentation such as a letter from the alternate insurance carrier or the employer stating the student's name, insurance carrier's name and policy number and the loss of coverage date via our Contact Form

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

Fall Academic Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in during the Fall Academic Enrolment period.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

Accessibility Accommodation Opt-In

If you're enrolled part-time, but meet certain accessibility requirements, you can submit the opt-in form up to 30 days from your program start date.

You must be a part-time student registered with Student Accessibility Services as a full-time equivalent student. You can reach out to your Plan Support Office for more information, or have your Student Accessibility Services Advisor submit your full-time equivalent letter on your behalf.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

  1. All eligible students are automatically enrolled in the Student Health and Dental Plan and do not need to submit an opt-in form. You will see the CUESA Health Plan Fee and the CUESA Dental Plan Fee on your student account to confirm you are enrolled.
  2. Students choosing to enroll in the health, dental or both plans.
  3. Approval of the Student Benefits Plan enrollment application will result in the CUESA Health Plan Fee and/or the CUESA Dental Plan Fee being assessed on your student account.
  4. NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
