Fall Application Period

September 16th to September 29th, 2026 at 4:00 PM

Loss of Coverage Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in up to 30 days after losing your alternative coverage.

Before completing this form you must provide documentation such as a letter from the alternate insurance carrier or the employer stating the student's name, insurance carrier's name and policy number and the loss of coverage date by contacting your Plan Support Office.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

Fall Academic Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in during the Fall Academic Enrolment period.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

Winter Academic Opt-In

If you're a new student starting your studies in the Winter Term, you may be eligible to opt in during the Winter Academic Enrolment period.

This deadline only applies to students who were not enrolled during the previous Fall term, and are enrolled and eligible for the current Winter term.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

  1. All eligible Fall students are automatically enrolled in the Student Health and Dental Plan and do not need to submit an opt-in form. You will see the Health and Dental Plan Fee on your student account to confirm you are enrolled.
  2. Students are required to enrol in both the health and dental plans; individual enrolment in either plan separately is not available.
  3. Approval of the Health and Dental Plan re-enrolment application will result in the Health Plan Fee and/or the Dental Plan Fee being assessed. You will receive an email informing you how to pay the fees.
  4. NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
