Loss of Coverage Re-Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in up to 30 days after losing your alternative coverage.

Before completing this form you must provide documentation such as a letter from the alternate insurance carrier or the employer stating the student's name, insurance carrier's name and policy number and the loss of coverage date via our Contact Form

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

Fall Academic Enrol

If you've previously opted out of the plan(s) you may be eligible to opt back in during the Fall Academic Enrolment period.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

May Enrolment Opt-In

If you're a new student starting your studies in May, you may be eligible to opt in during the May Enrolment period.

This deadline only applies to students who were not enrolled during the previous Fall term, and are starting a new program beginning in May.

If this deadline is missed, you will not be eligible to opt in again until the next Fall Academic Enrolment period.

  1. The Student Health Plan is mandatory, therefore, there is no Health Plan enrollment.
  2. All eligible students are automatically enrolled in the Student Health and Dental Plan and do not need to submit an opt-in form. You will see the Health Plan - medical insurance premium fee and the Dental Plan fee on your student account to confirm you are enrolled.
  3. Approval of the Dental Plan enrolment application will result in the Dental Plan Fee being assessed. You will receive an email informing you how to pay the fees.
  4. NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
